Patient Care Team

During your stay at the hospital, the following members of your care team will work together to provide optimal care for your treatment and recovery:


Your attending physician is responsible for the management of your medical care. He or she oversees your care and orders the medications, tests, therapies and procedures you will receive. Consistent with our multidisciplinary approach to care, he or she may draw upon the expertise of any member of your patient care team in treating your medical condition. You and your family are encouraged to talk directly with your doctor, ask questions about your health concerns and participate in your care.

While at the hospital, you may be seen by a hospitalist rather than your own physician. Hospitalists specialize in caring for your acute needs and are available 24 hours a day, seven days a week. They maintain communication with your physician to ensure consistent care.


At the forefront of the excellent care provided at Holy Family Hospital is our nursing staff. In collaboration with your physician, the nursing staff will work with you to meet your health care needs while you are hospitalized. Your nurse manager is in charge of all nursing care on your unit. Upon admission, you will be assigned a nurse who will plan and coordinate your nursing care with you and your family. These registered nurses provide 24-hour comprehensive care tailored to your special needs. Please feel free to ask your nurse about any aspect of your care.

Allied Health Professionals

Nutrition, rehabilitation services, pharmacy, spiritual care, physical/occupational/speech therapies, respiratory services, radiology and other health professionals are an important part of your care team. They provide valuable information and assist you in the healing process to promote good health in your future.

Social Workers

At Holy Family Hospital we care for the whole person, both physically and emotionally. Social workers are available to counsel patients and their families. We encourage you to talk to them about any fears or stresses you may be experiencing. To contact a social worker, call extension 2405.

Patient Advocate

All staff members consider themselves representatives on your behalf. In addition, a designated patient advocate is available to make sure that your visits to Holy Family Hospital, either inpatient or outpatient, are well managed, and that any concerns you may have are handled in a timely fashion. To contact the patient advocate, call extension 2399.

Rapid Response Team

If you have a significant change in condition of an emergency, we have a rapid response team that can come to your bedside. Simply contact your nurse and request that the rapid response team be called.
